Damage reporting form for Winter Storm Quinn
Medford OEM is asking Medford residents, who’s homes or businesses sustained damage from the recent winter storm to complete the attached form so we can capture data that maybe used to request a Federal Declaration from FEMA. This form will also allow you to attach a photo of the damage.
